HASC ESC PROGRAM AY 2022-2023

Incoming Grade 7 students who wish to apply for ESC program of HASC shall follow procedure below and submit soft copy of the application to registrar@hasc.com.ph with Subject: ESC Application_Surname of applicant

• Slots are limited and shall be given to deserving applicants. Priority given to Grade 6 graduates from public schools.

• We are accepting ESC grantees incoming Grade 8, 9 and 10 from other private schools. Submit the soft or scanned copy of your ESC certificate upon application for enrollment.

Duration of Application: Enrollment period

End of Application: Monday, 15 August 2022

Guidelines:

1. Please fill out the ESC form. An incomplete form is invalid.

2. Kindly prepare the following requirements:

   1. 2 pcs. recently taken, identical 2x2 picture

   2. PSA Certified Birth Certificate

   3. Photocopy of Grade 6 report card (for incoming Grade 7 only)

   4. Grade 6 Form 138 or Report Card with grade of at least 75% for all subjects in all quarters and final average grade rating of at least 75%

   5. Annual Income Tax Return Certificate (ITR for 2021) of both parents (if employed)

   6. Certificate of Tax Exemption or Municipal Certification of Unemployment of Parents/Legal Guardian (if unemployed)

3. Send all scanned copy of your form and requirements through email with Subject: ESC Application_Surname of applicant at registrar@hasc.com.ph

4. Please wait for an ACKNOWLEDGEMENT email before submitting the hard copy of your documents personally or by courier.

5. The ACKNOWLEDGEMENT email will serve as confirmation that your application or documents submitted has passed the evaluation of the school ESC program screening committee.

6. After receiving the acknowledgement, you may then submit the hard copy of your documents to: HOLY ANGEL SCHOOL OF CALOOCAN, INC. EDUCATIONAL SERVICE CONTRACTING (ESC) Camarin Road, Brgy 173, Caloocan City 1400

7. Compliance is necessary to validate your ESC grant application.

8. Only those with complete requirements will be processed.

9. Fraudulent and Falsification of documents will disqualify a student and forfeit his/her chance to participate in the ESC program.
